Report completed by the Grama Niladhari of the division
Original and photocopies of documents confirming ownership of income and assets
If the income is submitted as the income of the head of the household, a letter from the Grama Niladhari confirming that there is no objection to using his income for the relevant matter

Bills transacted to prove income (three to six months) should include the name of the business

4) Transportation services
The book of the vehicle should be in the name of the person
Obtain the income certificate otherwise submit the letter of consent of the owner confirm the income by giving a letter to the Grama Niladhari regarding the monthly or annual income.

5) Other small businesses
Letter confirming employment from the employer
That the income obtained by the Grama Niladhari is earned and that the salary details are employed
Letter of Registration if the business is registered in the Divisional Secretariat
Certified copies of bank accountAll the copies should be certified by the Grama Niladhari as authentic copies
